## Description

The Kingston and Richmond Proactive Anticipatory Care \(PAC\) model aims to ensure people can be better supported at home, by preventing deterioration and increasing independence in people with escalating health and social care risks\. PAC is an MDT model of care which introduces new roles \(Care Coordinators, Locality Coordinators and, PAC Managers\) to support cross-organisational working around patients' individual needs
