Description
The Council wishes to commission a cost effective Home from Hospital Transport, Settlement and Support service to support people with discharge from hospital and prevention of admission & readmission to hospital. The aim is to enable service users to return home safely in a timely, dignified manner and to regain their confidence after a period in hospital or a short stay in A&E in a way that will help to promote their independence, reduce dependency on intensive home care packages, and reduce the likelihood of hospital readmission or long term care. The budget for this service is approximately £240,000 per annum. The contract will be awarded initially for 1 year and 3 months with a possibility to extend for 1 further year, depending on performance and availability of funding. Transfer of Undertakings (Protection of Employment) Regulations 2006 (TUPE) may apply to this contract. If your organisation wishes to participate in this process, you need to register with the Buckinghamshire Business Portal and follow the instructions. You be unable to take part if you do not register. The tender return date is midday on Friday 11th August 2017. Tenders received after this time and date will not be accepted.
