Stroke Recovery Service including Communication Support

Description

Stroke Recovery service is designed to provide the right support at the right time to ensure every stroke survivor makes the best recovery possible. The Stroke Recovery Service will support all adults diagnosed with a stroke, their family and carers with high quality Information, advice, emotional and practical support. The service will also support people diagnosed with Transient Ischaemic Attack (TIA). The Communication Support service will provide flexible support to adults and their families and carers whose communication is affected by stroke, following their discharge from the acute health setting.
