Care and Support in two Existing Supported Living Schemes in Goole

Description

The Council is looking for a care and support provider for two independent supported living schemes in Goole. The landlord will be remaining in situ, this tender is only for the care and support. Each of the two schemes can accommodate a maximum of six residents each and more information regarding the specification is included in Schedule 1 along with redacted pen profiles of each of the current residents. Some of the redacted care plans and the TUPE information are available upon completing and returning the confidentiality agreement via the messaging function on YORtender. All the residents have identified needs and care is required to be delivered by a registered CQC provider with experience in supporting individuals with learning disabilities, in particular those approaching or over retirement age.
