## Description

This project aims to help the Office of Product Safety and Standards \(OPSS\) assess the feasibility of creating a concise, yet effective questionnaire for the collection of Harms Data\. Harms Data encompasses information about injuries, their causes, and the resulting consequences, including both fatal and non-fatal outcomes\. In addition to the initial 21 week term \(contract end date - 06/04/26\), an optional 8-week contract extension may be exercised solely at the discretion of the Buyer\.
