## Description

In April 2013, the Health and Social Care Act 2012 set out Local Authorities statutory responsibilities for commissioning the NHS Health Check programme\. Cornwall Council are inviting all GP Surgeries within Cornwall and the Isles of Scilly to join a light touch, Approved Provider List for the Provision of Health Checks\. Service Users registered with the Service Provider who meet the criteria \(Service Specification 2\.2\.1\) should be invited to attend a face-to-face NHS Health Check once every five years\. The "Invitation and Offer" can be sent by letter, email, or SMS\. Service Providers can also offer opportunistic appointments\. The "Invitation and Offer" should meet the Accessible Information Standard\. The eligibility criteria are that the Service User must be registered with a GP Service Provider within Cornwall and Isles of Scilly and consequently all Providers looking to join the Approved Provider List will have to be a registered GP within Cornwall and Isles of Scilly\.
