## Description

Bracknell Forest Council is looking for a suitable provider to deliver a Public Health Nursing service \(Health Visiting and School Nursing\) to improve the health outcomes and reduce inequalities for the 0-19-year-old \(25 for Special educational needs and disability \(SEND\)\) children, young people and their families in line with the updated Healthy Child Programme \(HCP\) national model and guidance \(2021\)
