Home from Hospital Service

This service is intended to support any resident of Bracknell Forest, aged 18+, requiring additional support on discharge from hospital. The service will liaise with key stakeholders to receive and action referrals. People will be supported for a short period of time with both visits and telephone calls to support emotional needs, small practical tasks and relevant signposting and guidance. The service is not expected to provide personal care. Contract term - initial 2 year period with two optional 12 month extensions (i.e.2+1+1 years)
