Description

Purpose To implement a digital prevention gateway to support Housing Solutions by; • Improving homelessness triage and early intervention • Enabling residents to access financial support (grants, benefits etc) • Reducing demand on frontline services Scope The solution must provide a Hybrid model (refer & accept) • Residents can self-serve to find and apply for support • MKCC can configure application pathways for internal schemes • Officers can support residents and apply on their behalf Users • Residents (self-serve) • Officers/support workers (assisted journey) • MKCC staff (admin portal for review and decision-making)
