## Description

Purpose To implement a digital prevention gateway to support Housing Solutions by; • Improving homelessness triage and early intervention • Enabling residents to access financial support \(grants, benefits etc\) • Reducing demand on frontline services Scope The solution must provide a Hybrid model \(refer & accept\) • Residents can self-serve to find and apply for support • MKCC can configure application pathways for internal schemes • Officers can support residents and apply on their behalf Users • Residents \(self-serve\) • Officers/support workers \(assisted journey\) • MKCC staff \(admin portal for review and decision-making\)
