## Description

The NHS Health Check Programme aims to support eligible adults aged 40-74 to identify and reduce their risk of developing cardiovascular disease \(CVD\)\. The Health Check involves several measurements and lifestyle questions, the results of which inform a behaviour change conversation and lead to clinical follow up where necessary\. Medway Council are responsible for ensuring the statutory obligations of the NHS Health Programme are met within Medway\.
