Procurement of Strategic Partner on behalf of the Doncaster Health and Social Care Community to support the development and implementation of the Placed Based Plan

Description

The Doncaster Health and Social Care community appointed a Strategic Partner via a competitive call off process via the CCS Consultancy 1 Framework. The partner is appointed to advise, facilitate and guide our local partnership in order to make the place plan vision and supporting governance mechanisms a reality. The Strategic Partner will undertake the "journey" with us, will develop a number of specific products and outputs in the next 12 months and will potentially develop a longer term relationship with the Doncaster Health and Social Care community. The Strategic Partner needed significant demonstrable knowledge and experience in the field relating to new care models for health and social care, with a track record of utilising cutting edge research and best practice. For example this could include experience of working with Pioneer or Vanguard communities, or experience of working internationally on this agenda. The Strategic Partner is expected to help build upon the work already undertaken in Doncaster and the current position with regards to the commissioning and provision landscape.
