## Description

City of York Council and the NHS Humber & North Yorkshire Integrated Care Board \(the "Commissioners"\) are looking to commission dedicated Discharge to Assess \("D2A"\) unit with up to 10 beds on a block purchase basis to support our patients and customers following discharge from hospital on discharge Pathway 2\.
