## Description

Southwark CCG are looking to commission a Diabetes Structured Education Booking Hub on behalf of all 12 CCG's in South London\. The hub will enable an easily accessible and flexible route for Health Care Professionals to refer patient with Type 1 and Type 2 Diabetes to Structured Education\. It will also have the facility to allow patients to self-refer\. The aim is to reduce some of the barriers people face in accessing Structured Education, maximizing use of available resources and patient choice\. The hub will enable patients to attend any course offered in South London so they can choose a day, time and location that best meets their needs\.
