## Description

Cornwall Council is commissioning services that deliver community based day support\. The contract will be outcome focused\. This means that Cornwall Council will not be prescribing a specific delivery model\. Providers will need an operating model that will achieve outcomes that are appropriate for the individual attending the service\. Cornwall Council aims to commission services that deliver better outcomes for individuals, and recognise that the service needs to be flexible and adaptable in order to meet this effectively\. Providers need to demonstrate how their service will deliver against the four central outcomes: 1\. Ensuring quality of life for people with care and support needs 2\. Delaying and reducing the need for care and support 3\. Ensuring that people have a positive experience of care and support 4\. Safeguarding adults whose circumstances make them vulnerable and protect them from avoidable harm
