## Description

Sunderland City Council is undertaking an Evaluation of its Integrated Wellness Service \(Live Life Well Service\)\. This is in order to ensure the services meet Public Health Outcomes, this will include undertaking a process evaluation and an outcome evaluation of its commissioned service called the Live Life Well Service which delivers on key elements of the Integrated Wellness model\.
