Description
The Trust requires a provider to support the identification of key priorities relating to health inequalities through community engagement. This supports the delivery of the Community Mental Health Framework for Adults and Older Adults (CMHF) and the NHS Long Term Plan's vision for a place-based, community mental health model. Enabling modernisation of community services to offer whole-person, whole-population health approaches, aligned with the new Primary Care Networks.
