Phase 5 of the pan-London CHC in nursing homes AQP contract

Description

Phase 5 of the pan-London CHC in nursing homes AQP will open for applications in June 2016. Successful providers will be added to an AQP list which will be used by London Clinical Commissioning Groups (CCGs) and discharge teams to make continuing healthcare placements in nursing homes. The AQP is the standard route of commissioning for the participating CCGs. This means that care packages will be commissioned with AQP providers.
