Delivery of Phase 1 of South West London Health and Care Partnership Interoperability Programme on behalf of NHS Wandsworth CCG

This work is the first phase of South West London (SWL) Local Digital Roadmap which will support the implementation of the new models of care and transformation schemes. It will be the first stage of SWL - wide information sharing across borough boundaries which will support future information sharing initiatives and will support the Sustainable and Transformation Plan's aims to reduce non-elective admissions, improve hospital productivity, and support the delivery of coordinated general practice at scale.
