Description
The aim of the Service is to provide short term, intensive, floating Housing Related Support for young people and young parents aged 16 to 25 years old living in the community with various types of properties regardless of the tenure. The floating support should not last more than two years. Housing Related Support will enable Customers to gain the skills, knowledge and experience to secure and/or maintain their own accommodation and live independently, thus preventing, reducing and/or delaying their need for care and support through homelessness. The aim of the Service is to provide short term, intensive, Housing Related Support lasting not more than one year for Customers aged 25 years and above. The service support Warwickshire customers to stay in their own homes or work towards living in their own homes. In doing this the Provider will provide support to prevent homelessness, hospitalisation or institutional care.
