## Description

This Service is for the provision of support and structured activities for adults with a diagnosis of a learningdisability aged 18 years plus who are eligible for services from Adult Social Care, City of York Council\. TheService shall be delivered by the Provider at Burnholme Health and Wellbeing Campus in the Centre atBurnholme building \(The Centre\)\. The Service will be described as the "Base"\.
