The Central London Community Healthcare Trust (the "Authority") is issuing this selection questionnaire ("SQ") to parties wishing to apply for admission to the Assistive Technology, Associated Products and Accessories dynamic purchasing system ("DPS") under which the Authority may purchase assistive technology and associated products as well as accessories for these types of products.
