Description
Wakefield Council has developed a new model of provision for Housing Related Support, designed around: * Identifying the right services for people the first time * Delivering long term solutions to a person's homelessness, with as few transitions as possible * A Making Every Adult Matter (MEAM) approach - offering coordinated interventions that respond flexibly to meet assessed need. * A focus on harm reduction, recovery, resilience and independence; and * Promoting positive approaches to health and wellbeing. The model is comprised of 3 integrated pathways (detailed below) accessed through a single point of access. * Single Point of Access * Assessment Pathway * Access to Housing Pathway - Main Provision - Specialist Provision * Housing Sustainment Pathway. The scope for this procurement is the Complex Needs Service which forms part of the Specialist Provision Pathway. Key aims of the Service will be: • To promote independent living skills and increase the service users ability to manage and maintain a tenancy. • To focus on harm reduction, recovery and resilience • To support service users' to address their drug and alcohol issues by working in partnership with appropriate specialist support services. • To prevent re-offending • To create opportunities for change and progression • To promote positive approaches to health and wellbeing. The service will support people living in 8 units of accommodation at Springhill Close in Outwood, Wakefield. The duration of the contract detailed is for the initial term of the contract. The Council reserves the right at its sole discretion to extend the contract by up to a further 3 years.
