Cardiovascular Disease Prevention Programme

To commission a Cardiovascular Disease (CVD) Prevention Programme.

This service will target population groups with unrecognised risk factors for CVD that are not being identified through the NHS Health Check programme. The cost per case identified will be lower than that of a Health Check and is aligned with the practice income from Quality and Outcomes Framework payments. For example, practices will carry out a full health check on those identified with high blood pressure with no additional payment from DCC.
