Domiciliary Care Urgent/Same Day response service

The urgent response service will provide a same day response for hospital discharges or where there is a potential admission avoidance within the community. The service will work alongside Intermediate Care Services offered by both health and social care in the system. The aim of the service is to maximise flow through the hospital for winter, ensuring people do not stay in hospital longer than needed
