## Description

The Council wishes to appoint providers to support hospital discharge for people who require a care home setting\. For people who require bed-based 24-hour care: includes people discharged to a care home for the first time\. Those discharged to a care home for the first time will have such complex needs that they are likely to require 24-hour bedded care on an ongoing basis following an assessment of their long-term care needs\. This block contract is for a maximum of 5 x Pathway 3 \(complex nursing care\) beds per provider There will be a maximum of 3 nursing complex placements per home at any one time Additional information: Please note that Walsall Council has a new Contract opportunity\.
