## Description

To provide NHS Health Checks to people aged between 40-70 with particular focus on those identified with increased risk of developing cardiovascular disease \(CVD\)\. These include current smokers, people aged over 50, people with mental health problems and people living in deprived areas\. Additional information: This contract is awarded for an initial period of two \(2\) years, with an option to extend for a further period/periods of up to six \(6\)\.
