## Description

The service is to deliver a person centred, personalised programme of support lasting between 3- 15 months to vulnerable adults which is reablement focused and to develop the skills and confidence to achieve optimum independence level and move on into appropriate long-term accommodation\. For a period of: 24 months with an option to extend a further 12 months
