Work description

The Family Weight Management (FWM) Service will deliver an outcome focussed, Family-based, flexible and Multi-Component lifestyle weight management Service for Children, Young People and their Families. The Service is to be part of a community-wide multi-agency approach to promoting a healthy weight, preventing and reducing obesity.

The Service will be delivered across Staffordshire and support overweight and obese Children and Young People (CYP) to reach and maintain a healthier Body Mass Index (BMI).

The FWM Service will use evidence-based Family focused weight management interventions, utilising Best Practice and professional experience of successful weight management interventions. Taking a Whole Family Approach to support CYP and their Families who are motivated to realise and maintain a healthy weight.

It will promote healthy weight and healthy lifestyle choices by focusing on:

Whole Family nutrition and healthy eating habits. Physical activity and the reduction of sedentary behaviours; supporting Families to access community provision such as local parks and leisure facilities to enable a sustainable continuation after completion of the intervention. Providing strategies for behaviour change for CYP and Families. Working with CYP and Families to achieve goal -based outcomes, to achieve a healthier BMI and sustain their behaviour change beyond the intervention. Providing Information, Advice and Guidance (IAG) and signpost to available online self-help tools for use by the Whole Family.

The Provider will use innovative approaches to meet the needs of CYP and Families and be required to evidence and evaluate the approaches taken.

In line with current NICE guidelines, the Service will provide 1:1/ face-to-face and group sessions comprising of peers, parents or carers. Sessions can be face-to-face, digital or via telephone support, reducing barriers to accessibility and engagement with the Service.

The Service will provide health and wellbeing promotion, to support healthy weights and healthy behaviours, preventing problems from occurring or getting worse.

The Service will be engaged with and support the Staffordshire-wide Better Health Staffordshire partnership, working as part of the Whole Systems Approach to tackle the causes of excess weight and promote healthy weight, active lifestyles and healthy eating.
