## Description

Middlesbrough Council requires a supplier to complete and review Education Health and Care Plans in line with SEND Code of Practice 2014\. The successful service provider will be responsible for writing and completing quality assurance for all plans which will be written and reviewed\. They will also ensure information provided by the Council is included and plans are submitted within specified timescales as set out in the service specification\.
