Work description

This is a Voluntary ex Ante Transparency Notice in Pursuant of Regulation 99 of the Public Contract Regulations (2015)

The aim of the pilot scheme is to develop new pathways for elderly people who have a high dependency on formal care and are at risk of hospital admission. The scheme relies on significantly enhancing the role of the voluntary sector, to focus interventions on what the individual, their carer, their family and the community can do for themselves.

This early intervention service would enhance the role of the voluntary sector to give them a more formal role within Care Coordination teams, with staff and volunteers conducting guided conversations with residents to help identify their needs/aspirations, feeding this information into MDTs to ensure that non-clinical needs are understood and responded to.

The service will help professionals' discharge/de-escalate residents from their caseload and allow clients who do not meet the eligibility criteria for formal care to receive 'low-level' interventions from the voluntary sector such as support with dog walking, arranging for minor aids & adaptations, assistance around the home and being escorted to social events.

It is now well established that these non-clinical interventions can prevent deterioration in health conditions and delay the need for formal care, as well as significantly reducing admissions to hospital. In addition, the interventions have been shown to improve quality of life and empower individuals to take an active part in their community - which reduces social isolation and increases wellbeing.

Additional information: A 10 day standstill period Commencing on 28th of April and finishing on Monday 8th of May 2017 is now in force.
