NEPRO2 - Transforming Adult Social Care - Front Door project support\r \r project_3713\r contract_6188\r \r provide the project management support for the Front Door workstream (including preparation of steering group documentation), and further build on this, continuing to develop the programme plan, and supporting progress & risk reporting. \r \r Front Door TOM - the previous phase developed a revised operating model and high level structure for the Careline teams. To take this forward, ASC need to undertake detailed design of how the teams will be structured and operate under the new TOM, including the implementation of the LCC's layers and spans principles. \r \r The SPS provider will support ASC and HR Leads with this through Project management,, Business analysis and data analysis roles to complete the detail design over the 12 week period, with activity to design and agree\r \r ? Teams - develop the revised staff profiles, responsibilities and structures that support the Front Door TOM. This work will require engagement and support from LCC HR/OD resources. \r ? Information - data required to inform and direct activity of the teams and how performance and outcomes will be measured\r ? Technology - utilising existing and planned technology products \r ? The quantitative and qualitative measures for evaluation and ongoing performance monitoring.
