## Description

1\. Background; The Camden Integrated Care Service \(CICS\) was set up in April 2012 as a pilot with the aim of addressing system challenges\. These challenges highlight the need for; - Different health and social care services need to be designed to be delivered in an integrative collaborative manner in order to maximise health outcomes - More patient focussed methods of delivery - Delivery of specialist and more integrated support in the community CICS aims to provide patients with access to secondary care clinicians in a community environment\. Central to the service model are clinics bringing specialist clinicians to treat patients in a community setting and weekly MDT meetings specifically for COPD patients\. CICS aims to offer practices and patients a responsive service that can be locally tailored to patient needs with the potential to manage co-morbidities in the same location and at the same time, with cross cutting services on site to further support individual needs\. CICS referrals can be made for patients with: • Chronic kidney disease \(CKD\) • COPD \(and COPD MDT\) • Heart failure • Frailty • Memory • Epilepsy 2\. The Clinical Review; The aim of the clinical review is to; - Understand whether the services within CICS are meeting the needs of the patients and outcomes anticipated of CICS - Identify the challenges and benefits of delivering integrated service and services in the community - Highlight the relevant trends \(to include service delivery and impacts on local health economy\) - Identify learning and best practice to inform the Camden Local strategy and other transformation projects - Make recommendations to inform the future commissioning of each of the CICS services and utilisation of the centre space\. - Gather and assess views of clinicians and patients on the service 3\. Approach / Methodology It is expected that the review will use the following information/ data sets • Community EMIS data gathered from internal reports, including monthly outcomes dashboards and progress reports and reviews/research • Secondary Care data ‘linked to GP registered populations SUS activity data – inpatient and outpatient activity • GP referral data • Community services data • Care Pathways per service • Estimated cost of existing services using relevant activity data from secondary care and community care \(e\.g\. Community Respiratory Team\) • Service specifications and service level agreements • Budget and spend for CICS project • Overview of local strategies and other transformation projects which may have some interdependences • Provider Quality Report – Quality Reporting of Camden Commissioned Services to the Quality and Safety Committee\. Additional information: We expect this support to be provided by an organisation that can provide a team of experienced individuals with a proven track record of working on assignments of this type and have previous experience of supporting NHS organisations to review services\. Successful bidders will be able to evidence the relevant knowledge, skills and resources to deliver what is required\. These may include but are not limited to: • Relevant experience and expertise involving the review or evaluation of services or projects\. • Knowledge and expertise of Integrated Care Models • Knowledge of community services and models of care involving moving specialist care into the community\. • Access to a Consultant and a General Practitioner to lead and support the clinical review\. • Ability to make both clinical and non-clinical recommendations around the service model and view of each service within CICS\. • Experience and knowledge of NHS commissioning\. • Previous examples of working with NHS organisations to deliver a detailed review, work involving integrated services or pilots is desirable\. • Expertise in different contracting models and ability to analyse complex data sets across GP and secondary care services\. Ability to assess capacity and demand to understand utilisation\. The provider organisation must have the capacity, skills and expertise to manage and deliver this proposal and engage key stakeholders in the London Borough of Camden where necessary\. Providers must be able to mobilise quickly with all capacity in place within 4 weeks of the award date\!
