Social Care Digital Innovation Programme

Local Government Association (LGA) programme aimed at addressing problems in health and social care through the application of digital innovation. The contract is for additional resource to complete the requirements of the programme within the programme timeframe. This encompasses a Discovery Phase using User Centred Design methodology to examine the issues that care homes in Leeds experience when a transfer of care takes place from a residential care setting to a hospital.
