Work description
Delivery of the following elements in the Community Family Hub in the specified geography within the East of the city- Newcastle upon Tyne. Community Family Hub lead role, Support for families with children pre birth to five years in identified Super Output Areas (SOAs) as part of the Sure Start Children's Centre (SSCC) offer. Intensive whole family support for families with children up to age of 18 with additional needs.
