Dementia Support - Kensington & Chelsea

To support the care of people affected by dementia through 3 projects: • Age Home Respite • Dementia Volunteer Co-ordinator & Advisor • My Memories Café Service

Additional information: This is an existing service that has been in place since April 2020. The CCG has assessed that the service needs to continue whilst a review is completed by the newly formed North West London CCG from April 2021.

The existing contract is being modified through contract variation to extend its term by 6 months. The existing value of the contract of £124,154 will increase by £62,077 during this additional term.
