CHC Digital Replacement System for NHS Luton Clinical Commissioning Group

The NHS Continuing healthcare service supports people over 18 years old who have been assessed as having complex ongoing health needs. If eligible, the NHS will fund a patient's care needs. CHC is a complex 'end to end' process and the digital tool assists automating the workflow process to improve patient service and boost productivity in the Continuing Healthcare area. The Solution provides management and financial oversight as well as supporting the reduction of staff needed to operate and deliver the service. IT functionality enables the CHC Team to meet the Agreed Service levels set out by NHS England.

Additional information: The Crown Commercial Service (CCS) G Cloud 11 Framework Contract was used to identify the "best fit" provider (Care Banking) who could meet the needs of NHS Luton Clinical Commissioning Group Continuing Health Care Team and service requirements. The initial contract duration is for a period of 24 months with an option to extend further for an additional 12-month period in accordance with the parameters of the CCS G Cloud 11 Framework agreement.
