Work description
The City and Hackney Public Health Team seek to award a contract for a Youth Justice Nurse Service via Direct Award C under the Provider Selection Regime. The Youth Justice Nurse is a role within the Hackney healthcare system which is responsible for the management of referrals and delivery of comprehensive health assessments.Referrals are made directly by the members of the multidisciplinary Hackney Youth Justice team to the Youth Justice Nurse Service. All Children and Young People that have been allocated by Youth Justice Team Leaders will be given an appointment to see the Youth Justice Nurse. The Youth Justice Nurse completes a comprehensive individual health assessment for children and young people in contact with the Hackney Youth Justice Service or following their release from a secure setting. This will ensure that the health needs of children and young people are identified through the delivery of high quality and holistic health assessments which consider physical, sexual, emotional wellbeing and mental health, cultural needs and health promotion.
