## Description

The contract is required to provide 6 nursing step down beds to support people with enhanced/complex needs from the acute hospital beds\. We are anticipating the model to be a phased approach, this will however be determined by the demand during December 2023/January 2024\. The phased approach will continue during January to a maximum of six beds\. The support delivered will need to be person-centred and outcome based and the monitoring of the contract will be through a fortnightly meeting\. People will be discharged from Great Western Hospital seven days a week with a requirement to use the trusted assessor to aid swift discharge within 12 hours\. The initial contract term will be for 3 months \(13 weeks\) with an option to extend for a further 4 weeks for a potential total contract term of 4 months \(17 weeks\) as stated in the ITT documents\.
