Work description
To provide a highly responsive, safe/high quality and robust OOH base and home visiting service for the registered, unregistered and resident population of West Kent. Ensuring patients receive the right care at the right time, right place, with better support for people to self-care. Reduction of ED attendances, emergency admissions and unnecessary ambulance conveyances
