Work description

The Council is looking to establish a new five-year(if all extensions utilised) fixed framework contract for the provision of Support to Live at Home (STLAH) within the local authority area. The framework is to be established to provide social care funded packages of support to adults aged 18 and over, who meet the eligibility criteria. This includes adults who have assessed care and support needs associated with older age, physical and sensory impairment, mental illness, learning disability and for people living with dementia. Adults relevant to both levels of Service include: · Older People, including older people experiencing mental and emotional distress and with a diagnosis of Dementia. · People with a physical disability and sensory impairment, including people living with long term conditions. · People experiencing mental or emotional distress. · People with a learning disability. · People with Autistic Spectrum Disorders (ASD) · People living with HIV/Aids. · People with end-of-life care needs (i.e. in last months, weeks and days of life). · Vulnerable adults Please note an option will be detailed in the invitation to tender to re-open the fixed framework should providers leave the framework or if demand for the service increases. Please note that this advert is to replace the Support to Live at Home flexible framework. Please, note the contract start date is the date that the Council is aiming to issue out the contracts for Round Two of Support to Live at Home, the original framework commenced on the 1st February 2024.
