South West London Integrated Urgent Care

Description

South West London Health and Care Partnership (SWL) are seeking to engage with providers and gauge market interest to commission an Integrated Urgent Care (IUC) Service consisting of GP Out of Hours (OOH), 111 and Clinical Assessment Service (CAS), further supported by close working links with acute emergency departments and nearby community services. The provider will be required to deliver the service through a highly integrated, safe and cohesive service model, reducing unnecessary hospital attendance or admissions and enabling optimal patient outcomes. The specification will outline the Commissioners requirement for the provision of services. The contract term is anticipated to commence on 1st April 2020 for a period of 5 years with the option to extend for a further 2 years.
