Work description
To provide an accessible and clinically effective, community based service for the assessment, diagnosis and treatment of defined ophthalmic conditions, which equals or exceeds the quality of comparable services provided by secondary care. To improve the patient experience and reduce the number of onward referrals to secondary care To operated a well-planned and clearly articulated care pathway, delivering safe and evidence-based care, supported by a robust clinical governance framework To ensure that referrals are appropriately assessed and that patients are seen by the most appropriate clinician with the relevant skills, right equipment, in a suitable location To comply with all relevant waiting times standards and ensure that all stages of the patient’s care are delivered in a time-frame suitable to the patient’s clinical, emotional and social needs To offer services in 2 or more locations, easily accessible to the population of South Gloucestershire
This is a 36 month contract with the possibility to extend for up to 24 further months subject to commissioners discretion.
