Single Homeless Prevention Service

Description

This contract works with single non-priority homeless people who do not meet the 'priority need' criteria under homelessness legislation. The contract will be outcomes based and payment will only be made based on results achieved to prevent or relieve homelessness and sustain any housing outcome. The results which attract payments are: • Completion of a Personal Housing Plan for a customer • A successful prevention or relief of homelessness, resulting in the customer having accommodation which is predicted to be of at least 6 moths' duration • Evidence that the accommodation outcome has been sustained for 8 months Full details of the services required can be found in Document Part C - Specification
