## Description

Wakefield Council has developed a new model of provision for Housing Related Support, designed around: \* Identifying the right services for people the first time \* Delivering long term solutions to a person's homelessness, with as few transitions as possible \* A Make Every Adult Matter \(MEAM\) approach - offering coordinated interventions that respond flexibly to meet assessed need \* A focus on harm reduction, recovery, resilience and independence; and \* Promoting positive approaches to health and wellbeing\. A procurement process has already taken place for the Assessment Pathway, Access to Housing Pathway -Main Provision and the Housing Sustainment Pathway\. This procurement is for 2 of the Access to Housing -Specialist Provision\. The 2 services include; Young Families Service; and Supported Housing for People with Mental Health Needs\.
