## Description

The purpose of the service is for the service provider will seek to assist adults \(18 years and over\) with an identified \(functional\) mental health problem within a recovery model\. The service provider will be closely linked into local mental health services and will work in collaboration to ensure a coordinated approach to managing the client's mental health\. Supporting positive outcomes around employment, education, housing and wider social inclusion\.
