## Description

Midlands Partnership NHS foundation Trust \(MPFT\) is looking to procure providers who are able to provide a care and support at home\. These Services forms part of a suite of Integrated Prevention and Discharge to Assess \(D2A\) 'Home First' services across Staffordshire and Stoke on Trent and include provision of Reablement service \(upto 6 weeks\), short term interim home care \(pre/post reablement\) and night sits\. Tenders are welcomed from Providers interested in one or more of these requirements\.
