## Description

The London Borough of Camden is seeking to commission a new Carers Support Services Contract\. The service supports identification of Carers, increased access to a range of support including advice, information, statutory Carer assessments and peer support to enable Carers to remain connected to their community\. The new service will reflect the voice of Carers and embody the principles of the action plan\. The service will be based around a neighbourhood footprint in line with the developing health and care integrated neighbourhood teams and neighbourhood networks\. The proposed service is for an initial term of four years with an option to extend for three years, up to a maximum of a seven-year period\. The break clause would allow for the service to be reviewed to ensure that services are working in line with the changing landscape of health and social care and that the contract continues to meet the needs of Carers in Camden\. The contract value for the proposed service is £6,314,798 including extensions\. The deadline for the tender is 6th December 2024, 12\.00pm midday
