NCC NHS Health Checks IT System

Description

The NHS Health Check programme aims to reduce avoidable premature mortality by early identification and management of cardiovascular risk factors and disease in people aged between 40 and 74 years who have no previous history of CVD disease. In Northamptonshire, 75 in every 100,000 people die prematurely before the age of 75 from cardiovascular disease. An IT solution is required to support the operational process and monitor the performance and effectiveness of the NHS Health Check programme at all levels. The purpose of the IT support system is to enable health check providers to effectively carry out the NHS Health Check programme and for local authority commissioners to effectively manage and quality assure the programmes processes and outcomes. The service will be required from 1st April 2019 for an initial period of 1 year (up to 31st March 2020) with an option to extend for a further period of up to 2 years, taking the total contract term to a maximum of 3 years. Any extension will be agreed between the Council and the Provider. The deadline for completed ITTs to be received on the LGSS eTendering Portal is 12:00noon on 26th November 2018.
