## Description

Wigan Council would like to digitise some of its Adult Social Care functions, namely Reablement and In-House Care and Support Services, by way of the introduction of a digital social care record for the purpose of electronic care planning\. Partial funding from Health Innovation Manchester/GMCA has been secured to facilitate the project delivery\. The target is to complete the implementation by the 3rd June 2024 to realise the full benefit of the funding\.
